How to Request a Certificate of Insurance:
Information required in order to process request…
- Insured’s Name
- Certificate Holder (company certificate is issued to)
- Physical Address of Certificate Holder
- How do you want the certificate delivered? (fax, mail or e-mail)
- Provide the information needed for deilvery (fax #, e-mail address, & mailing address if different that the certificate holder address)
- Any Special Requests Needed for Certificate (special requests are not guaranteed until the requestor has received verbal confirmation from Hemby Insurance. In some cases, the insurance company has to grant permission for requests.)
- Requestor’s Name & Contact information in case of questions
